Below, are some of the main services we offer that we can discuss in more detail during our visit. Hopefully, they provide a general overview, assisting you and your relatives to choose what best suits you.

SUPPORT & ADVICE

The Office Team are able to ‘sign post’ you to relevant information to enable you to solve difficulties or provide you with the details of advocacy support services that may be able to help.

This service helps and supports you to exercise your rights and makes sure your voice is heard. We can point you in the right direction if you require any help with the following:

  • Welfare benefits
  • Housing and Property Matters
  • Respite care
  • Social Care
  • Health organisations
  • Social Activities
  • Personal matters

HOSPITAL DISCHARGE

Most people feel safer and more comfortable in their own home. We acknowledge this and can arrange care and support following a hospital stay. We will always conduct an re-assessment of your care needs  and design a person-centred care plan before a safe discharge can take place.

MEDICATION

Our fully trained staff can administer your medication if you are unable to do this independently or can help you to take your medication from a chemist filled ‘ Blister pack’ or from ‘original packaging boxes or bottles.

If your medication requires a specialist technique , we can allocate a support care worker trained in the specialist knowledge required and your care/support plan will be agreed with others involved in your health care, such as your district nurse who will assess your support care worker and sign them as competent.

SHOPPING SERVICE

If you have problems accessing a supermarket. Our Support Care Workers can either go to accompany you to the shops or do the shopping for you.

Being accompanied gives you an opportunity to socialise over a cup of tea and a snack!
